PPIVMS AND PAIVMS

Hi All!

I'm currently on my msc sk prac and am having to do alot of Ax of PPIVMS and PAIVMS in the Lx, Thx and Cx region. I am getting really frustrated that I cannot pick out the segment/s that are most restricted. No matter how hard I try, or how much I think "maybe I can feel this segment is stiffer" I am never sure. My supervisor then comes in and picks out all these levels that are 'more resticted' and they are most often not the ones that I have nominated. It really disheatens me becasue if I can't do it by the end of the prac what am I supposed to do once we start full time work if I am working in the msc sk area!?

I have been told that research now days shows that you should treat multi levels as oppossed to one anyway, but it still disheartens me that I cannot "feel it". Is any one in the same boat? Or was any one the same as me but have found something to asssit them in being able to feel the most resricted segments?

Hi Kate,

I had difficulty myself trying to figure out which segment was stiff. But I find it easier now when you got left versus right, top vs bottom to compare. It's really hard for any of us students, as we do not know what "normal" feels like. But give it another go. Maybe I'm finding it easier cause my patients might be skinnier :P. Pple have reported having to second guess when patients are large.

Keep trying and I'm sure you'd get it!

Hi Kelly,

I am completely with you. I absolutely dread performing the technique because I feel so hopeless with it. Like you, I like to now definetly what segments are involved and then I feel much more confident treating it. I have dealt with this problem by basing my findings more on pain production and subjective response to my pressure rather than just going on joint restriction alone. After treating the joints that are indicated to me through this tecnique, my patients often report to feeling better so I have taken to using this alot. In saying this, over time my confidence is improving, so I suggest to keep trying and hopefully you'll start to get a bit of positive feedback too.

Hey Kelly,
Don’t stress, I couldn’t feel which areas my supervisor said were more restricted either. I could feel a general stiffness in a broad area ie. c4-7, but not break it down the way my supervisor could. But guess what?!? She’d been working as a Musculo physio for 15 years!! We’ve been ‘working’ as physios in the musculo area for 4 or 5 weeks. So don’t stress! The more hands on experience you have, the easier things will get (well I hope so, because I’m definitely in the same boat as you!!) Ezza
